If you’re looking to change your Open Path therapist, please submit this form. Please note that you will need both the email address associated with your membership as well as your Member ID number to proceed.
Upon submission, a revised Member ID email will be automatically sent to your email address on file. Please check your spam folder if you don’t see it in your inbox. Once you receive your revised Member ID email, you are welcome to contact your new therapist to set up your first appointment.
If you don’t know your Member ID number or encounter any difficulties with the form, please write to our team at registration@openpathcollective.org.
